We worked with HNECC PHN to develop a Primary Care Workforce Framework that supports better planning and investment in allied health and practice nursing. Our work combined data analysis, stakeholder engagement and strategic synthesis to guide future workforce development.
Engaging consumers and stakeholders to design Healthy Hearts Healthy Minds program for COORDINARE
We were engaged by COORDINARE to deliver stakeholder engagement to inform the design of the Healthy Hearts Healthy Minds program. We worked with consumers, general practices and partners to gather insights on patient experience and service design priorities.
Assessing health needs of people experiencing homelessness for Murrumbidgee PHN
We were engaged by Murrumbidgee PHN to complete a regional health needs assessment focused on people experiencing homelessness. The work supported place-based planning by identifying barriers to care, service gaps, and opportunities to improve access and coordination.
Review of after hours homeless healthcare service for Northern Queensland PHN
We supported NQPHN to review and re-commission a specific homeless health care service that provided mobile outreach and clinic-based health services for people experiencing homelessness.
Developing a 3-year Palliative Care Action Plan for Brisbane South PHN
We were been engaged to undertake the development of Brisbane South PHN’s Palliative Care 3 Year Action Plan. This involved a review of the findings and outcomes of Brisbane South PHN’s previous palliative care projects to inform future commissioning decisions.
