Engaging consumers and stakeholders to design Healthy Hearts Healthy Minds program for COORDINARE

Background
COORDINARE was designing and commissioning the Healthy Hearts, Healthy Minds program to address cardiovascular disease (CVD) risk, a priority health condition in the South Eastern NSW region. The program involves a nurse-led model of multidisciplinary care delivered within local general practices.
We were engaged by COORDINARE to engage with consumer representatives and participating general practices to inform the design of the model of care and establishment of this new program.
Approach
We gathered insights through two online focus groups with consumer representatives, a wider online survey for community members, and a workshop with participating practices and other expert partners. Our facilitation involved a journey mapping approach to introduce the proposed model, identify how it might meet the needs of the target uaudience, and gather feedback on opportunities to strengthen the model, including what it offers, who it reaches and how it’s delivered.
Outcome
We analysed qualitative insights and reported back to COORDINARE with considerations to refine the service model — exploring communication strategies, workforce capability, holistic care and practice support. These findings helped to shape the establishment of the Healthy Hearts Healthy Minds program. You can check out a summary of findings from the process here.
