Service and project design - chronic disease, mental health and AOD, place-based initiatives: CQWBSC PHN

Client: Central Queensland, Wide Bay, Sunshine Coast PHN (CQWBSC PHN)

Background

Central Queensland Wide Bay Sunshine Coast PHN commissioned Beacon Strategies to undertake a number of Central Queensland based service design activities. Support provided by Beacon Strategies supported the PHN to develop and implement a number of new service offerings.

Outcomes

Beacon Strategies delivered a number of pieces of work under the one consultancy arrangement:

  • Development of a chronic disease project plan, inclusive of

    • Understanding the chronic disease needs and trends of the region through existing needs assessment documents

    • Understanding the suite of available programs and services to leverage

    • Conducting stakeholder consultation where appropriate (GPs / community organisations)

    • Conducting evidence review (academic literature and grey literature)

    • Exploring options across the continuum of care inclusive of prevention, early intervention, and treatment.

  • Development of a Mental Health and AOD program plan, inclusive of:

    • Understanding how CQWBSC-PHN is situated in terms of reform process/agenda

    • Mapping currently funded services to particular funding streams to better understand the funding environment

    • Understand reporting required for DoH e.g. what is urgent vs. longer-term planning

    • Prioritise ‘quick wins’ vs. long term strategy

    • Identify areas across funding streams that may require consultation or service design

  • Development of a project plan for a potential place-based initiative for Woorabinda, inclusive of:

    • Enabling an issue identification process led by the community (empowerment focus, foster self-determination)

    • Fully understanding the demographics and population-level health status of the community

    • Conducting stakeholder consultation where appropriate to deepen the understanding of the area, focusing on relationship development.

    • Co-designing a service response that is ‘fit-for-purpose’ for the local community
