Background
As identified by Brisbane North PHN, the overall goal of the Care Pathways Project was for:
… people in the Brisbane North PHN region to get the right care in the right place at the right time when they are at risk of suicide, following a suicide attempt, or bereaved by suicide.
Progress toward the overall goal was achieved through the development of a clear, organised and connected suicide prevention clinical care pathway across and within the Brisbane North PHN region, which has high utility across sectors and enhances the care and support for people at risk of suicide and for people who require aftercare/follow-up after a suicide attempt, or who are bereaved.
Approach
The following objectives were achieved as a result of the Care Pathways Project.
Increased stakeholder and community knowledge and understanding of the various pathways to care within the PHN region, if someone is at risk of suicide, has attempted suicide, or is bereaved by suicide.
Increased stakeholder and community knowledge of the specific pathways to care in the region for Lesbian, Gay, Bisexual, Transgender and Intersex (LGBTI) people, and Aboriginal and Torres Strait Islander people
Increased stakeholder and community knowledge and understanding of the systems in place to respond to and provide care to people at risk of suicide, in the PHN region.
Improved partnership with the Metro North Hospital and Health Service (MNHHS)
Improved knowledge of current best practice Emergency Department guidelines (when working with a person who is suicidal), to be released by the Taskforce and identify implementation factors.
Development of an innovative co-design approach, which builds on previous work by the PHN, national and international approaches to suicide prevention work.
Development of the care pathways into a product/s with high utility across sectors (hospital staff, service providers, GPs and community members)
